Support for people with special needs

Community Partners Program (CPP)

The Community Partners Program (CPP) aims to promote and facilitate increased and sustained access to aged care information and services by culturally and linguistically diverse communities with significant aged care needs.

The primary outcomes of the CPP will include:

• More aged care support services delivering culturally appropriate care to older persons from culturally and linguistically diverse communities;
• Older people from culturally and linguistically diverse communities having increased access to culturally appropriate age care support services;
• Older people from culturally and linguistically diverse communities having greater capacity to make informed decisions about aged care support services.

Guidelines for the CPP program have been drafted to assist in the development and management of CPP funded projects. They provide details of the program, eligibility, accountability and roles and responsibilities and are available on Community Partners Program (CPP) Guidelines.

Community Partners Program 2009-2010

The Community Partners Program (CPP) provides grants to community based organisations representing culturally and linguistically diverse communities. These organisations use these grants to provide support for older people from culturally and linguistically diverse communities in accessing aged care information and services.

Partnerships with Division of GPs, Carer Resource Centres, Carelink and Carers NSW to provide information sessions and training for carers.
Cambodian-Australian Welfare Council of NSW Inc	South West Sydney Aged Care Planning Region	Cambodian	This project will promote awareness and facilitate increased access to aged care services. The project will target the Cambodian (Khmer) community in the Fairfield, Liverpool and Bankstown statistical local areas in the South West Sydney aged care planning region. Key activities will include: Development of information; networking, conducting cultural information sessions, holding forums to canvas the needs of the Cambodian community.

Fraternal Society of Tripoli and Mena Districts Ltd	South West, Inner West, South East and Western Sydney	Lebanese, Syrian, Egyptian, Iraqi, Sudanese, Somalian, Jordanian and Palestinian	The project will support aged care service providers in the four target regions to design culturally appropriate service delivery that caters to the specific needs of the Arab Muslim communities.

Indonesian Welfare Association Inc	Western Sydney, Northern Sydney and South East Sydney	Indonesian	The project will promote aged care services and facilitate increased access for the older Indonesian community in the Northern Sydney and Western Sydney regions, with proposed expansion to South East Sydney; a website with translated aged care brochures will be developed; key partnerships with the Indonesian community will be maintained; a demographic profile of the Indonesian community will be developed and included in cultural briefings to aged care services.
Italian Social Welfare Organisation of Wollongong (IT.SO.WELL)	Illawarra	Italian, Spanish	The project will assist to reduce access barriers for the Italian speaking elderly by continuing to develop and strengthen aged care service links and partnerships; commence developing links and partnerships for the Spanish speaking elderly.
Macarthur Diversity Services Inc	South West Sydney - Macarthur areas	Arabic (Lebanese, Egyptian, Iraqi, Turkish), Spanish (South American, Spanish), French, Filipino (Tagalog), Vietnamese, Samoan, Indian (Hindi, Urdu), and Chinese (Mandarin) communities.	The project will involve development of links and partnerships with aged care providers and palliative care services; development of best practice models of care for CALD communities. Provision of community information sessions and cultural briefings to support and resource aged care services to provide culturally appropriate care; and development of formal relationships with key state and local services to assist CALD communities to better access services.
Macedonian-Australian Welfare Association of Sydney Inc	South East Sydney, South West Sydney, Inner West, Nepean, Illawarra	Macedonian	The project will inform older people and their carers on aged care services through workshops in local areas across the target regions and using the Macedonian media; provide cultural briefings to residential aged care providers where there are clusters of Macedonians.
MigrantLINK Australia Ltd	Northern Sydney, Western Sydney	Afghani (Dari) and Iranian (Farsi)	This capacity building project targets elderly Afghani and Iranian refugees in the Northern Sydney, South West and Western Sydney regions. The project will increase the provision of support to older members of these communities and enhance their access to aged care services by raising awareness of the type of aged care and support services available in these regions and by developing sustainable partnerships with service providers.
The Polish-Australian Welfare Association Inc (PAWA)	Hunter, Illawarra, Inner West, Northern Sydney, South East Sydney, South West Sydney and Western Sydney	Croatian, Czech, Hungarian, Polish, Serbian and Slovak	The project will assist older people and their carers from the Czech, Slovak, Polish, Croatian, Hungarian and Serbian communities access aged care services, information and resources in the Hunter, Illawarra, South East Sydney, South West Sydney, Western Sydney, Inner West and Northern Sydney regions. It will expand community education activities and utilise multicultural media outlets to disseminate information.
Queanbeyan Multilingual Centre Inc	South East NSW including Queanbeyan, Cooma, Goulburn, Young and NSW South Coast - Ulladulla to Moruya and Moruya to Eden	All CALD Communities	This project will identify CALD community information needs regarding aged care services across the South East NSW region and provide an information and access model relevant and appropriate to CALD communities across the region.
The NSW Spanish and Latin American Association for Social Assistance Inc (NSW SLASA)	South West, South East and Western Sydney aged care planning regions.	Spanish speaking communities	The project will provide support and provide information on aged care support services to elderly in the Spanish-speaking communities in South West, Western and South East Sydney regions; it will be community focused and will develop partnerships and communication to bridge the gap between the community and aged care services.
Sydney Multicultural Community Services Inc	Inner West and South East Sydney Regions	Maltese, Portuguese, Croatian, Spanish, Tagalog, Filipino, Serbian German, Fijian Indians of Hindu background	The project will develop, implement and evaluate a multicultural communication strategy targeting the Maltese, Portuguese, Croatian, Spanish & Latin American, Filipino, German, Fijian, Indian and Serbian communities. Peak ethnic community organisations will also be involved. Will improve community knowledge and understanding of aged care services through provision of translated information, referral to services and training of community volunteers.
SydWest Multicultural Services Inc	Western Sydney Region	Arabic speaking communities from the Middle East and Tamil speaking communities from Sri-Lanka, India and Singapore	A 12 month project working with providers of Commonwealth funded aged care services and the elderly from Tamil and Arabic speaking communities across Western Sydney region. The project will deliver information and education on aged care services and issues to the target groups. Deliver cultural briefings, establish partnerships between aged care providers and the target communities, develop strategies to address work force issues by training staff, or assist in recruitment of bi-lingual staff to work with Tamil and Arabic communities.
Victoria
Alzheimer’s Disease and Related Disorders Association of Victoria - Trading as: Alzheimer’s Australia Victoria	Victoria wide: Finnish: Western Metro and Hume; Lao: Melbourne Metro; Romanian: Outer South Eastern Metro; Russian Jewish: Southern Metro	Finnish, Lao, Romanian and Russian	This projects aims to empower small ageing communities in Victoria that have minimal organisational support. A worker from each of the four nominated communities will work with Alzheimer’s Australia to develop resources and information about aged care.
Australian Croatian Community Services Inc	Metro Melbourne, Greater Geelong (Barwon Southwest), Ballarat (Grampians Region) and Latrobe region including Gippsland (Gippsland Region)	Croatian and Bosnia and Herzegovina	This project will strengthen and further develop the working relationships in Metropolitan Melbourne, and Greater Geelong, Ballarat and Latrobe including Gippsland; prepare an information package about the communities. Develop a range or programs, workshops, training and information sessions aimed at current and potential carers on dementia, mental health, diabetes.
Australian Greek Welfare Society Ltd	Local Government Areas (LGAs) of Darebin, Whittlesea, Moreland, Monash, Manningham, Kingston, Brimbank, Mildura, Shepparton, Greater Geelong, Northern Metro, Eastern Metro, Southern Metro, Western Metro, Loddon-Mallee, Hume, Barwon-South Western	Greek, Cyprian and Egyptian	The project aims to develop culturally relevant responses to the aged care and health needs of the ageing Greek-speaking community. These responses include access to information and access to culturally appropriate care.
Australian-Polish Community Services Inc	Victoria in general – State-wide. First year throughout the Metropolitan region i.e. all Statistical Local Areas (SLAs). Second year spreading to rural and regional Victoria – Geelong, Morwell, Ballarat	Polish	This project aims to expand the current scope of activities funded through the Community Partners Program to cover the entire state of Victoria. The first year involves working with Palliative Care and Respite /Carer support services in the South Metropolitan Region (SMR) and East Metropolitan Region (EMR). The second year involves prioritising services to rural and regional communities. The third year will see the consolidating of metropolitan and rural services established in the first 2 years to incorporate the whole State.
Australian Turkish Association Inc	Victoria State wide	Turkish	This project aims to promote aged care services to older people of Turkish-speaking background through cross-cultural training to partnered aged care services; development resources for aged care workers; referral pathways for older people of Turkish-speaking background to aged care services, community education program of aged care services and general support and community linkages for Turkish-speaking elders.
Ballarat Regional Multicultural Council Inc	All of Ballarat region	Dutch, Italian, Greek, Croatian and German-Austrian	This project will develop and introduce an Intercultural Peer Facilitator model within the City of Ballarat region; recruit 10 bilingual volunteers from existing Ballarat Regional Multicultural Council Inc volunteer groups to become intercultural peer facilitators. Training of facilitators on the impact of ageing, the structure and how to access aged care services, cross cultural communication, the role of advocacy, group facilitation and consultation strategies.
Care Connect Limited	Victoria - Western Metro Region, Maribyrnong, Hobson Bay, Moonee Valley, Wyndham, Brimbank, Melton, Hume	Macedonian and Turkish	This project will identify and remove barriers to Aged Care; empower community through information on services; and, promote outreach community linking, community mapping exercise.
Centre for Philippine Concerns Australia Incorporated	Southern Metro Region: Casey, Cardinia, greater Dandenong, Kingston, Knox, Frankston and Peninsula Eastern Metro Region: Monash, Whitehorse, Manningham, and the Gippsland Region: La Trobe Valley, Sale, Bairnsdale.	Filipino	This project will provide cultural briefings to service providers, establishing innovative strategies and develop resources to inform providers about cultural sensitivities to ageing.
Chinese Community Social Services Centre Inc	Four metro regions in Melbourne: Eastern Northern, Southern and Western	Chinese	This project will ensure that Chinese-speaking communities will better understand and access aged care services and will assist community and residential aged care providers to provide more culturally appropriate service.
City of Greater Bendigo	Victoria - Loddon Mallee region; City of Greater Bendigo, Central Goldfields, Loddon, Campaspe, Mt Alexander and Macedon Ranges	Dutch, German, Italian, East African, Filipino, Chinese, Serbian, Mandarin, Maltese	This project will provide information sessions on aged and disability services, residential facilities and respite care will be conducted throughout the Loddon Campaspe region. Women and men’s health days (Bendigo and Kyabram) and the Walking Group/information sessions in Bendigo will continue providing monthly information sessions.
Co.As.It. Italian Assistance Association	Victoria State wide	Italian	This project will provide information and assistance to the Italian-Australian community which will promote and facilitate access to aged care services; cultural briefings to aged care service providers and other relevant organisations including care givers so that they can meet the needs of the Italian community; assistance to aged care services to provide culturally and linguistically appropriate aged care services to Italian older people.
Diversitat - Geelong Ethnic Communities Council Inc	City of Greater Geelong and Borough of Queenscliff	Italian, Bosnian, Dutch, German, Croatian, Polish, Greek, Ukrainian, Macedonian, Slovenian, Hungarian, Serbian, Spanish, Maltese, Austrian, Indian, Chinese, Sudanese	This projects aims at bridging the gap between culturally and linguistically diverse communities and aged care services. Aged care services will be resourced to provide culturally appropriate care.
Filipino Community Council of Vic Inc	Metro regions throughout Victoria	Filipino: Tagalog, Visaya, Kapampangan, Bicolano and Cebuano	This project is a long term ‘State-wide Neighbourhood Model’ through partnership/alliance with mainstream agencies. The Filipino Community Partners Program explores creative and innovative ways to deliver culturally responsive programs that will enhance the access of the Filipino community to aged care information, options and services at the same time build the capacity of partner agencies (mainstream and residential) to deliver appropriate care to present and future Filipino service users.
Fronditha Care Inc	Metro Melbourne and Mornington Peninsula	Greek	This project will work with Carers VIC to develop a program to educate and aged care staff and increase cultural awareness and sensitivity.
Gippsland Multicultural Services Inc	Victoria Local Government Areas (LGAs) Regional / Rural - East Gippsland Shire, Wellington Shire, Latrobe City, Baw Baw Shire, South Gippsland Shire and Bass Coast Shire	Italian, Dutch, German, Tagalog, Ukrainian, Greek, Maltese, Polish, Croatian, Serbian, Spanish, Hindi, Chinese	The Multicultural Aged Liaison Group represents all the key CALD communities in the Gippsland region. The project will provide this group with information on aged care issues and seek their input on CALD related issues. The group will disseminate the information to their communities. The project will provide information sessions on a range of aged care services and issues. Aged care services will be provided with a range of information, resources and training to develop culturally responsive services to the CALD communities.
Indochinese Elderly Refugees Association Vic Inc	Cities of Brimbank, Maribynong and Greater Dandenon.	Vietnamese	This project will provide culturally appropriate information to aged care services and remove cultural barriers; enable the Vietnamese community to make informed decisions to accessing aged care services; provide visits/tours to relevant aged care facilities to help increase knowledge in and change the attitude towards aged care services among Vietnamese people; and develop in English and Vietnamese information resources.
Macedonian Community Welfare Association Inc	Victoria State wide, northern and western local government areas of Whittlesea, Brimbank, Darebin, Maribyrnong, Hobsons Bay and Moreland	Macedonian	This project will work to remove the cultural and language barriers preventing the Macedonian community from accessing and utilising aged care information and services; provide clients with a social support network in times of hardship while educating service providers about how to work with the Macedonian community in a culturally sensitive way.
Maltese Community Council of Victoria Inc	Victoria - Metro and regional Gippsland	Maltese	This project aims to improve culturally appropriate aged care in residential and respite services; will organise Maltese language workshops, educational sessions in partnership with Carers Victoria; and formulate resource development and visits to facilities.
Migrant Information Centre (Eastern Melbourne) Ltd	Eastern Metro, Boroondara, Knox, Manningham, Maroondah, Monash, Whitehorse and Yarra Ranges	Armenian, Chinese, Egyptian Coptic, Hungarian, Macedonian, Maltese, Russian, Vietnamese and small communities of Indonesian, Japanese, Korean, Lithuanian, Spanish speaking Ukrainian, Turkish, Burmese and Sudanese	This project will provide information on aged care services, palliative and carer support targeted to CALD communities. North East Multicultural Association (NEMA)	Moira, Alpine, Wangaratta, Wodonga	Italian , Croatian, Dutch and German	The Aged Migrant Connection Project will build upon its successes from the last 18 months by further strengthening the working relationship between the volunteer Cultural Peer Facilitators and the aged care sector beyond the current level of service; extending the reach of the Aged Migrant Connection Project into more CALD communities; developing the expertise of the Project .
Regional Information and Advocacy Council Inc	Victoria - Hume Planning Region, Greater Shepparton, Shire of Moira, Rural City of Benalla and Shire of Strathbogie	Greek, Italian, Albanian, Macedonian, Dutch, Turkish and Arabic speaking communities	This project will enable a range of ethno-specific and other agencies to increase awareness of care options among CALD communities.
St John of Kronstadt Russian Welfare Society Inc	Melbourne, Morwell and Geelong Metro area	Russian, Belorussian, Ukrainian, Serbian, Moldovan, Georgian, Polish, Macedonian, Slovenian other former USSR	The project will distribute information in kit form, through Powerpoint presentations, in forums and through radio programs to the Russian Community on Aged care options. Provide information sessions to Aged Care Assessment Service (ACAS) medical and other providers of aged care on the cultural needs, customs and beliefs of the Russian speaking community; and assist aged care workers by distribution of communication aids such as cue cards and language charts for contact with non-English speaking people.
Serbian Welfare Association of Victoria Inc	Metro Melbourne, and regional areas of Geelong, Wodonga and Yallourn	Serbian	This project aims to increase access and information related to aged care in the Serbian Community; establish relationships between community and aged care services; enhance knowledge of aged care services about the Serbian culture and target rural Serbian communities.
South Eastern Region Migrant Resource Centre Inc	South eastern region - Greater Dandenong, Casey and Kingston	Sri Lankan and Cambodian	This Sri Lankan and Cambodian project will educate older people and their families about the range of aged care services and enable them to make more informed choices about care. Sri Lankian and Cambodian communities will be targeted for 18 months each. Cultural briefings and resources will be provided to aged care providers and advice and assistance given to providers.
Spanish Latin American Welfare Centre Inc - CELAS	Melbourne Metropolitan Region	Spanish	This project will provide information and support for elderly Spanish speaking people, their families and carers.
Spectrum Migrant Resource Centre Inc	Hume, Whittlesea, Darebin, Banyule, Moreland, Nillumbik and Yarra	Somalian, Turkish, Chaldean, Eritrean, Punjabis – (African communities)	The project will work with main stream age care providers to focus on African communities and new and emerging small ethnic communities to provide information on and facilitate aged care services.
Springvale Indo-Chinese Mutual Assistance Association Inc	South eastern region - Greater Dandenong, Casey and Kingston, Monash, Frankston, Cardinia	Vietnamese	This project aims to promote and facilitate a range of activities and programs for the Vietnamese elderly community to assist them access aged care support services, to bridge the gap between the elderly and aged care services and assist the elderly exercise their rights and make informed decisions regarding aged care services.
Sunraysia Mallee Ethnic Communities Council Inc (SMECC)	Mildura / Sunraysia, Robinvale, Swan Hill	Afghan, Turkish, South Pacific Islands, Greek, Italian, Croatian	The project will introduce information to the communities from Greece, Italy, Croatia, Afghanistan, Sudan, Samoa and Turkey, information that will empower them to make informed decisions about aged service in the rural area of Mildura and Swan Hill. Over the 3 years information on Carer support, packages EACH, CACPS, EACH dementia and RACF will be given to the communities through information sessions. A Booklet of local aged care services will be produced and translated into the different languages.
Victorian Arabic Social Services Inc	Victoria – North West Metro, Southern Metro, Barwon-South Western, Hume	Arabic speaking communities and including those speaking Assyrian, Chaldean, Armenian, Kurdish and Turkish	This project is designed to support the provision of more culturally specific aged care services; increase access to aged care services for elderly people from the Arabic speaking communities and assist the target group to access information to support informed decision making in relation to aged care services.

The Chung Wah Association Inc	All metropolitan areas of Perth	Chinese, Vietnamese, Cambodian and other small Asian communities	The Chung Wah Link project will be achieved through the strategies of education, training, community building and partnership linkage with mainstream aged care service providers. The project will involve a series of educational activities eg seminars, exhibitions, recruiting and training volunteers, and information talks, and will involve joint partnership activities with Commonwealth Care Link.
Fremantle Multicultural Centre Inc (Formerly: South Metropolitan Migrant Resource Centre)	South-West Metropolitan region	Lebanese, Iraqi, Turkish, Egyptian, Indonesian, Iranian, Afghanistan, Italian, Croatian, Portuguese	This project will assist CALD communities and individuals in south metro regions to be better informed about the range of available aged care services and how to access them through the provision of written material and meetings.
Italo-Australian Welfare and Cultural Centre Inc	Metropolitan Perth and South-West WA	Italian	This projects aims to promote and facilitate increased and sustained access to culturally and linguistically appropriate aged care support services by aged members of the Italian community with significant aged care needs.
The West Australian Association of Polish Women Inc	Western Australia State wide	Bosnian, Bulgarian, Croatian, Czech Republic, Hungrian, Latvian, Lithuanian, Macedonian, Serbian, Polish, Slovakian, Romanian, Slovenian, Ukrainian	This project aims to continue a holistic and practical approach by delivering aged care information to people of CALD background and supporting aged care providers by developing a publicity strategy to present aged care as friendly by utilising ethnic and mainstream media; informing ethnic communities about aged care; providing support to aged care services and other health professional.
St Basil’s Aged Care Service in Western Australia (Vasileias) Inc	Perth and metro region of South-West Western Australia	Greek, Serbian, Russian, Romanian	This project will extend services to the Serbian, Russian and Romanian communities.
Tasmania
Migrant Resource Centre (Southern Tasmania) Inc (CPP)	Tasmania State wide	Sudanese, Ethiopian, Egyptian, Croatian, Bosnian, Macedonian, Slovenian, Dutch, Iranian, Afghanistan, Indian, Fijian and the Filipino	The project aims to increase and sustain access to aged care services for several CALD communities through the facilitation of bicultural workers and broad sector partnerships.
Polish Association in Hobart Inc	Tasmania State wide	Polish	This project will facilitate access to aged care services for the Polish Community aged care providers through information and consultation sessions.
Migrant Resource Centre (Southern Tasmania) Inc - PICAC	Tasmania State wide	All CALD communities	The project will convene regular Partners in Culturally Appropriate Care – Community Partners Program forums and organise workshops to maximise the impact of the Community Partners Program and avoid duplication of effort. It will provide opportunities for bringing aged care service providers and Community Partners Program organisations together to facilitate the development of relationships that will improve access to aged care services for older members of CALD communities.
Northern Territory
Carers NT Inc	Darwin and greater Darwin, Katherine, Tennant Creek Alice Springs	All CALD communities	This project will conduct information sessions for CALD seniors in major populations centres in the NT; develop written information with local knowledge; will use their existing framework to provide forums to allow CALD seniors to articulate unmet needs; promote Commonwealth Respite and Carelink Centres and Carer advisory Counselling services; organise training for community resource persons; conduct workshops for aged care support workers and liaise with interstate Partners in Culturally Appropriate Care, Community Partners Program and CALD organisations for multilingual resources.
Australian Capital Territory
ACT Chinese Australian Association Inc	ACT	Chinese	The project will build on the Chinese aged care information service to provide information in various formats in order to raise awareness in the Chinese community of aged care services available; continue to recruit volunteers to work with the Chinese aged care information service and to visit aged care services.
ACT Health - CPP	ACT	All CALD communities	The Project will recruit and train volunteers from CALD communities to be able to provide information regarding aged care services within their CALD groups and develop CALD aged care newsletters for volunteers and stakeholders.

Projects

Projects funded under the Community Partners Program (CPP) will provide aged care support services to older people from culturally and linguistically diverse communities to assist them to gain access to residential and other aged care information and services.
